Make sure you get the mileage repayment you deserve by taking the following steps:
Submit your completed mileage voucher to the Department of Labor and Industries or Self-Insured Employer within 1 year of the trip.
Make sure the trip is more than 10 miles from your home; the Department won't pay for trips less than 10 miles. They check the mileage using a computer mapping program.
Get your claims manager's written permission IN ADVANCE to utilize the doctor if there is a doctor with the same specialty within 10 miles of your home. Department rules permit them to deny travel reimbursement if there is a medical provider closer to your home in the appropriate specialty, EVEN if they have been paying in the past. The Department may make an exception if the closer doctor is not taking patients, or if not taking your particular medical problem. This requires a telephone poll of local doctor offices. You have a right to use the doctor of your choice; but you do not have a right to get paid to travel to any doctor of your choice.
If your mileage request is denied, you must protest it in writing within 60 days.
Make sure you submit your mileage voucher within 1 year of the travel.
The DLI reimbursement rates change from time to time. As of January 1, 2010, the personal vehicle mileage rate will be $.50 per mile. From January 1, 2009, the DLI mileage rate is $.55/mile. From July 1, 2008 to December 31, 2008, the rate was $.585/mile. January 1, 2008 through June 30, 2008, the DLI mileage rate was $.505 cents/mile, up from $.485.
